Earn them All. Earn them Early.

Make your health a priority in 2018! CHS LiveWELL Incentives reward teammates for maintaining or improving their physical, financial and personal health. All teammates can participate in the Incentives regardless of benefit-eligibility, however only teammates enrolled in the medical plan at the time of the payout processing date will receive the financial rewards.

Fund your Health Savings Account (HSA) with Incentives by earning up to:

  • $750 (Teammate only medical plan)
  • $1,050 (Family medical plan = insure at least one family member)

Here is How to Get Started:

  • Health Survey – complete this short, health questionnaire to learn about your health risks and opportunities for improvement. If you completed a health survey in the past, you will be able to view your health trends. Get started on the Total Health Portal
  • Health Coaching – after you complete the Health Survey, schedule time to speak with a health coach; or participate in a health coaching alternative, including CarolinaCare One-on-One Rx prescription consultation and many other alternatives
  • Financial Health – take advantage of tools, education and resources to help you be a more informed healthcare consumer
  • Know Your Numbers – knowing important lab values can help you maintain or improve your health
  • Healthy Weight Reward – maintain or achieve a healthier weight and waist measurements
  • Family Health Incentive – be healthy together with your family

Please keep in mind the CHS LiveWELL Incentives are paid to teammates who are on the CHS LiveWELL Health Plan (medical insurance) at the time of payroll processing who have an active 2018 HSA.

The Core Four Incentives you earned in 2017 were reflected on your January 12, 2018 paycheck (February 2018 for teammates who are paid monthly). Pregnant, breastfeeding, underweight teammates or teammates with medical exceptions can complete a Healthy Weight Reward or Know Your Numbers Alternative/Exception Form to earn their reward.

**Labs need to be drawn by October 5, 2018, then take action with one of the Know Your Numbers options so this Incentive is shown as complete in Total Health Portal by the last day of Benefits Open Enrollment

Core Four

Earn the $50 Bonus when you complete the Core Four by their deadlines: Health Survey, Health Coaching, Know Your Numbers and Financial Health/Virtual Tools Incentives. Your HSA contribution for the $50 Bonus will be paid in January, 2019.

Complete the Core Four by January 31, 2018 to participate in the Extra Early Bird Prize Drawing. Don't miss you chance to win one of ten CHS jackets.

Early Bird Prize Drawing

You have two chances to win. Complete the Core Four Incentives by:   

  • January 31, 2018:  Extra Early Bird Prize Drawing: You will be entered to win one of ten CHS Jackets
     
     OR
  • June 8, 2018: Early Bird Prize Drawing: You will be entered to win a catalog prize

If you complete your first Health Coaching call (or alternative) by January 31, 2018 or June 8, 2018, you will qualify to receive an entry into ONE of the Early Bird Prize Drawings.

If you plan to mail your Know Your Numbers form, please ensure it is postmarked by January 31, 2018 or June 8, 2018 to qualify for an entry into ONE of the Early Bird Prize Drawings.
